	A credit/debit card is required to pay the $35 application fee online.
	Do not submit an online application if you have submitted an application within the past year. For submitting applications by mail or fax, download a printable USA application by clicking on the Undergraduate Application Form" below, then fax or mail to the provided address in order to avoid paying another application fee.

Transfer Students

	Former USA undergraduate students who have missed a minimum of one semester and want to reapply as an undergraduate student should use a USA Readmissions Application to avoid paying the $35 application fee.

	Applications for admission and all required supporting documents, such as transcripts and test scores, should be received by the following deadlines:
